Physical Therapist (PT)

This position requires 8-hour shifts from Tuesday to Saturday.

Job Description

We are seeking a dedicated Physical Therapist (PT) to support the treatment of illness and/or injury through physical exercises and therapies. The goal is to assist in the rehabilitation and restoration of body functions to promote maximum client wellness and independence. The role will involve developing therapy programs for patients of all ages with developmental deficits, physical injury, or illness, along with additional duties based on the facility's requirements.

Responsibilities

• Coordinate PT services, delivering direct therapeutic care and teaching rehabilitation skills to clients and caregivers to promote independent functionality and safety.
• Provide preventive and rehabilitative PT procedures tailored to individual client needs.
• Develop and revise plans of care based on initial assessments, ongoing evaluations, client history, diagnoses, and goals, including appropriate discharge plans.
• Apply treatments utilizing physical agents such as heat, water, and electricity, while operating relevant equipment.
• Document all skilled and medically necessary care thoroughly and accurately, ensuring compliance with Medicare, state, federal, and other applicable regulations.
• Maintain client and organizational confidentiality at all times.
• Participate in quality management and complete required in-service programs to uphold standards.
• Supervise Physical Therapy Assistants and Home Health Aides as applicable, according to relevant laws and agency policies.
• Engage in staff meetings, case conferences, and customer service programs to meet facility goals.
• Deliver high levels of customer service to all internal and external customers.

Required Experience / Certifications / Licensure

Appropriate licensure as a Physical Therapist is required. Experience in a similar setting is preferred.
