Job Description

The role of the Outpatient Physical Therapist involves the evaluation of assigned patients and the development, implementation, and modification of treatment plans tailored to the patient’s needs, goals, and physician direction. The therapist is responsible for guiding and delegating all discipline-specific therapy services for these patients, ensuring the design of functional treatment programs that promote effective community reintegration.

The therapist also plays a crucial role in educating patients, families, students, and co-workers, while complying with the organization’s mission and core values and upholding professional standards and codes of conduct.

Responsibilities

• Complete comprehensive evaluations for all assigned patients.
• Utilize clinical reasoning to design suitable treatment plans aimed at progressing patients toward discharge within the established length of stay.
• Document in medical records accurately and promptly, including documentation for goals, assessments, and letters of medical necessity.
• Ensure care is provided under signed, valid physician orders and within insurance approval for patients.
• Participate in the student program by mentoring and providing educational opportunities for students.
• Create an individualized professional development plan and pursue continuous education and self-development.
• Maintain professional licensure in good standing and adhere to ethical standards.
• Engage in performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Outpatient clinical experience of 6 months to 1+ year.
• Current CPR certification.
• Active and applicable state licensure in Physical Therapy.
