Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans based on patients' needs, goals, and physician direction. This role includes delegation and direction of discipline-specific therapy services, ensuring the design of functional treatment programs to facilitate effective community reintegration.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and develop individualized treatment plans and goals.
• Document patient care in a clear, concise, accurate, and timely manner, ensuring compliance with relevant guidelines.
• Provide clinical care under signed, valid physician orders, ensuring insurance approval as necessary.
• Participate in educational programs for students, providing a positive learning environment.
• Create and pursue individual professional development plans, engaging in continuing education and professional activities.
• Adhere to the State Practice Act and maintain professional licensure in good standing while completing continuing education requirements.
• Understand and apply performance improvement methodologies to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Minimum of 6 months to 1+ year of outpatient clinical experience is required.
• CPR Certification is required.
