Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. This role involves delegating and directing all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs to facilitate an effective return to community activities. The Physical Therapist will also serve as an educator for patients, families, students, and coworkers while performing other professional duties related to patient care. Commitment to compliance, professional standards, and organizational values is essential.

Responsibilities

• Completes comprehensive evaluations of assigned patients and develops appropriate treatment plans focused on progressing patients towards discharge within an established length of stay.
• Documents medical records clearly, concisely, accurately, and in a timely manner in compliance with state, federal, and clinic policies.
• Participates in student program assignments, providing a positive learning environment and appropriate supervision.
• Creates an individualized professional development plan, pursuing professional growth through continuing education and involvement in professional activities.
• Maintains professional licensure and adheres to the discipline-specific State Practice Act.
• Understands organizational performance improvement methodologies and contributes to quality service improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months to 1+ year of outpatient clinical experience.
• Current CPR Certification.
• Active and applicable state licensure for practicing as a Physical Therapist.
