Registered Nurse - Hospice Coverage

Schedule

Monday to Friday from 8 AM to 4 PM, with some weekends, on-call, and holiday coverage required.

Assignment Duration

This is a 13-week contract position.

Job Description

We are seeking a dedicated Registered Nurse to provide hospice coverage. The ideal candidate will be responsible for delivering high-quality nursing care, developing care plans, and managing patient cases in compliance with all relevant regulations and best practices.

Responsibilities

• Effectively complete OASIS documentation in accordance with agency policies and state and federal regulations.
• Utilize the nursing process to assess patient and caregiver needs to provide quality care.
• Develop and communicate realistic plans of care, ensuring patient and caregivers are included.
• Serve as Case Manager for assigned patients while managing caseloads and visit times per agency standards.
• Implement medical plans of care and nursing interventions to ensure safe, effective patient care.
• Stay current on high-tech skills (e.g., IV use, wound VAC, complex wound care) and maintain infection control standards.
• Collaborate with LPNs and delegate tasks to ensure patient care plans are followed.
• Evaluate the effectiveness of care plans and document patient responses accordingly.
• Maintain accurate documentation of nursing care and ensure integration of information for the healthcare team.
• Follow agency protocols with McKesson processes such as visit lock-downs and completion.
• Provide patient and caregiver education on medication side effects, treatment options, and discharge instructions.
• Identify inter-disciplinary needs of the patient and communicate information to other disciplines in a timely manner.
• Accurately fill out agency, state, and federal forms as needed.
• Coordinate safe patient discharges and transfers, ensuring proper paperwork is processed.
• Advocate for patient care issues to address specific patient and family needs effectively.

Required Experience / Certifications / Licensure

• Valid RN license in the state of Vermont.
• Experience in hospice care preferred.
• Knowledge of OASIS documentation requirements.
