Description

Position: Case Manager
Location: Morristown, NJ
Pay Rate: $ 57/hr
Schedule: 8 AM - 4 PM

Duties:
-Coordinates patients through the acute care hospitalization Manages all aspects of discharge
-Planning/transition process Performs utilization management and quality screenings In- Patient Case Management is required

Requirements:
-2 years of acute care experience strongly preferred
-Bachelor's Degree Required
-RN license required
-CCMC preferred
-BSN required
-Case Management, Managed Care. In Patient Case Management is required
