Description

Position: Registered Nurse Hospice
Location: Basking Ridge, NJ
Pay Rate: $63/hr
Schedule: 8 AM - 4 PM

Duties:
-Performs Patient assessments: Accurately assesses patient's appropriateness for hospice care, learning needs and progress toward goals.
-Develops the plan of care for the patient and family
-Ensures appropriate utilization of services: Maintains visit productivity standards. Demonstrates knowledge of insurance reimbursement.
-Completes discharge assessments: Performs laptop transfer of data twice daily. Completes admission assessment and transfers within 24 hours.
-Assumes responsibility for Case Management: Develops/implements/evaluates plan of care based on evidence-based knowledge.
-Communicates progress toward goals with IDT. Attends and participates in IDT meetings every 2 weeks. Evaluates the plan of care and revises as needed.
-Accepting both travel or Local candidate.

Requirements:
-Must have two (2) years of experience as RN in home hospice environment.
-Experience using an EMR, excellent critical thinking and communication skills.
-3 Years of RN experience.
-Must have 2-3 years as RN in home hospice environment
BLS - required.
-Experience using an EMR, excellent critical thinking and communication skills
-New Jersey RN License Required.

Education:
-Associate's (ASN) degree OR Diploma with RN License in New Jersey.
-Current New Jersey RN License
-Must have valid New Jersey driver's license.
-BLS Preffered.
