Description
Position: Registered Nurse - Hospice
Location: Basking Ridge, NJ
Pay Rate: $ 55/hr
Schedule: 8 am - 4 pm
Duties:
-Performs Patient assessments:
-Accurately assesses patient's appropriateness for hospice care, learning needs and progress toward goals.
-Develops the plan of care for the patient and family
-Ensures appropriate utilization of services:
-Maintains visit productivity standards.
-Demonstrates knowledge of insurance reimbursement
-Completes discharge assessments:
-Performs laptop transfer of data twice daily.
-Completes admission assessment and transfers within 24 hours.
-Completes re-visit documentation and transfers
-Uses age appropriate teaching methods for patients and families:
-Evaluates teaching plan and revises as necessary.
-Documents patient education tools used
-Assumes responsibility for Case Management:
-Develops/implements/evaluates plan of care based on evidence-based knowledge.
Requirements:
-Must have 2-3 years as RN in home hospice environment
-Must have valid New Jersey driver's license
-BLS - required
-Current New Jersey RN License
-Experience using an EMR, excellent critical thinking and communication skills
