Description

Facility: Helen Porter Rehabilitation & Nursing

Approval for up to 32 weeks.

LPN for Helen Porter Rehab & Nursing: Will work 4 x 8s one week and 5 x 8s the next for 72H biweekly. A minimum of every other weekend and/or schedule adjusted to meet the needs of the unit and patient care. Floating required. Cat on unit - cannot be allergic. Questionnaire attached to position files needs to be reviewed with interested candidates and uploaded in system for IVS review. Travelers must work 2 out of the 4 holidays: Thanksgiving, Christmas, Christmas Eve, and New Years Day.

Requirements: • Dementia capable care certification preferred • A minimum of 1 year of experience • Prior travel required • EPIC experience from within the past 2 years is required and nonnegotiable

Additional Information: • IVS to OFFER • Monday to Sunday • No block schedules for any reason • Bi-weekly orientations on Monday, next available is 1/22 • Please ensure all employment gaps over 90 days are explained • This facility requires ALL work history information (bed count, EMR, facility type, etc.) to be complete in order to submit Disclaimers: • Public Transportation is NOT accessible in this area, please ensure you have reliable transportation, personal vehicle or rental vehicle are strongly recommended • Vehicles should be equipped with all season or snow tires during the months of November – April
