Facility: Home Health & Hospice
EPIC EMR PROJECT. Must be 1/12 start, 12 weeks.
MSW for Home Health and Hospice. Will be all 8 hour days but may also be expected to cross cover adult home health, hospice and community hospice as needed. IVS to review • Monday to Sunday • No block schedules for any reason • Please ensure all employment gaps over 90 days are explained • This facility requires ALL work history information (bed count, EMR, facility type, etc.) to be complete in order to submit Disclaimers: • Public Transportation is NOT accessible in this area, please ensure you have reliable transportation. Please note all providers must have reliable cars for the cold and snow to include snow tires; they must feel comfortable driving to patients homes in winter driving conditions
