Facility: Home Health & Hospice

Position Description:
NO TIME OFF IN MARCH, • Distance is measured by a straight line, not driving distance PT for Adult Home Health: CANNOT accommodate 10H shifts. Weekend rotations. This candidate will be expected to do 5-6 clients per day. We desperately need a candidate who has strong clinical/assessment skills and that is fluent in navigating different EMRs. Epic experience is required. Requirements: • Licensed PT, please note any others • Prior travel required, at least 2 previous travel assignments preferred • A minimum of 3 years of clinical experience (or more) with home health required Additional Information: • IVS to OFFER • Monday to Sunday • No block schedules for any reason • Bi-weekly orientations on Monday, next available is 12/01 • Please ensure all employment gaps over 90 days are explained • This facility requires ALL work history information (bed count, EMR, facility type, etc.) to be complete in order to submit Disclaimers: • tPublic Transportation is NOT accessible in this area, please ensure you have reliable transportation, personal vehicle or rental vehicle are strongly recommended • tVehicles should be equipped with all season or snow tires during the months of November – April
