Job Summary:

We are seeking a compassionate and experienced Registered Nurse (RN) to provide skilled nursing care to patients in their homes across Montgomery County and Northwest Philadelphia. The RN will be responsible for delivering high-quality, patient-centered care while promoting independence and improving patient outcomes in a home setting.

Key Responsibilities:

• Provide skilled nursing assessments and develop individualized care plans

• Administer medications, injections, and IV therapies as prescribed

• Perform wound care, catheter care, and other skilled nursing procedures

• Monitor and document patient conditions and progress

• Educate patients and families on disease management and home care techniques

• Coordinate care with physicians, therapists, and interdisciplinary team members

• Ensure compliance with home health regulations and agency policies

• Maintain accurate and timely documentation in EMR

Required Qualifications:

• Active Registered Nurse (RN) license – Pennsylvania

• Minimum 1 year of recent Home Health experience preferred

• Strong assessment and critical thinking skills

• Valid driver's license and reliable transportation

• Current BLS certification

• Excellent communication and documentation skills

Preferred Qualifications:

• Experience with OASIS documentation

• Knowledge of Medicare/Medicaid home health regulations

• Experience working independently in community settings

Work Environment:

• Travel required within assigned ZIP codes

• Combination of Montgomery County and Northwest Philadelphia territories
