Registered Nurse (RN) – Home Health

Location: North Princeton, New Jersey
License Required: Active New Jersey RN License

Position Overview

We are seeking an experienced Registered Nurse (RN) – Home Health to provide high-quality patient care in the North Princeton, NJ area. The ideal candidate will have strong OASIS experience and a background in high-volume home health settings. This role requires excellent clinical judgment, time management, and documentation skills.

Key Responsibilities

• Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans

• Complete OASIS assessments, Start of Care (SOC), recertifications, and discharge documentation accurately and timely

• Manage a caseload of approximately 5–6 patients per day

• Perform comprehensive patient assessments and develop individualized care plans

• Administer medications, wound care, IV therapy, and other skilled nursing interventions

• Ensure thorough and timely documentation in compliance with regulatory and agency standards

• Educate patients and families on disease management, medication management, and home safety

• Collaborate with interdisciplinary team members to coordinate patient care

Requirements

• Active New Jersey RN License (Required)

• Minimum 2 years of Home Health experience

• Strong and recent OASIS experience (Required)

• Experience working in a high-volume home health setting

• Ability to manage high documentation workload efficiently

• Excellent communication and organizational skills

Schedule Requirements

• Must be available to work 1 weekend out of every 4 weeks

Preferred Qualifications

• Experience with electronic medical records (EMR) in home health

• Strong time management and multitasking abilities
