Key Responsibilities:

• Provide skilled nursing care to patients in their homes.

• Complete accurate OASIS assessments and documentation.

• Perform wound care and manage wound VAC therapy.

• Develop and implement individualized patient care plans.

• Monitor patient progress and update care plans as necessary.

• Educate patients and families on disease management and care procedures.

• Coordinate with physicians, therapists, and other healthcare professionals.

• Ensure compliance with home health regulations and documentation standards.

Required Qualifications:

• Active Registered Nurse (RN) license.

• Minimum 2 years of Home Health RN experience.

• OASIS documentation experience (required).

• Experience with wound care and wound VAC therapy.

• Strong clinical assessment and communication skills.

• Valid driver's license and reliable transportation.

Preferred Qualifications:

• Previous experience working in community or home health settings.

• Familiarity with EMR systems used in home health.
