JOB DESCRIPTION
Speech Language Pathologist

● About Facility:
o Central office: 105 TR Harris Dr. Shelby, NC 28150
● Unit Information:
o Current census 197 for Cleveland
o Average 6 patients per day.
o Minimum Years of Experience required? 2 years (no CFY), would highly prefer Home Health experience, but will also take SNF experience
o Will you accept a first-time traveler? No
● Patient Types/Common Diagnoses:
o Geriatric/adult. Neuro, TBI, dementia, dysphagia, apraxia, aphasia, voice (limited)
● Required License, National Certification, Certs:
o State license, BLS, driver's license, insurance, vehicle, CCCs (certification of clinical competency, received after CFY)
● Skills required:
o Dysphagia, aphasia, cognitive treatment
● Support within the Department:
o Home Health
o Receptionist – at office
o RN/LPN
o Social Work
o PT/OT
o Scheduler
o Team assistant
● Technology/Equipment:
o EMR: HCHB – highly preferred
● Floating: If yes, where?
o Within branch or sister branch if availability allows. Will cover Gaston, Cleveland, and Rutherford Counties.
o Lincolnton Branch – if low volume for Cleveland, could float to assist with Lincolnton. About 25 minutes away
● Orientation (facility and unit):
o How many days of unit orientation? 5 days
● Scheduling:
o Weekend rotation: No
o On call? If so, what is that schedule? No
o Holiday Expectations: As needed for caseload
o Should approval of RTO go through manager or will you allow the interviewing team (if applicable) to approve time off on your behalf? Manager
o Shift times: 0800-1700, Monday – Friday. Manager open to 10h shifts if requested
