JOB DESCRIPTION
Position Title: Care Coordinator Social Worker
FLSA Status: Non-Exempt
Department: Care Coordination / Social Services
Reports To: Director, Care Coordination / Social Services Occupational Summary
Collaborates with patients (neonate through geriatric), families, medical staff, and community agencies to evaluate the appropriateness of hospitalization, determine optimal levels of care, and coordinate post-hospital needs.
Provides psychosocial assessment, crisis intervention, and support to facilitate effective care transitions and efficient clinical resource management. Primary Duties and Responsibilities
  • Screens cases pre-admission, admission, concurrently, and post-discharge in accordance with departmental policies.
  • Utilizes professional judgment to identify appropriate interventions impacting clinical resource management.
  • Collaborates with clinical staff to identify and prioritize discharge planning needs.
  • Gathers comprehensive patient information, including physical, psychosocial, financial, and support system factors.
  • Counsels patients and families to evaluate post-hospital care options and develop appropriate discharge plans.
  • Communicates with external reviewers and provides clinical documentation for reimbursement, including retroactive reviews.
  • Issues Notice of Non-Coverage letters in accordance with hospital policy and payer guidelines; discusses financial responsibilities with patients, families, and providers.
  • Coordinates with healthcare providers and community agencies to implement discharge plans.
  • Documents assessments, care plans, and progress in the medical record; communicates updates to patients, families, and care teams.
  • Refers cases to internal departments (e.g., Risk Management, Infection Control, Pharmacy) as appropriate.
  • Responds to referrals from financial counselors, physicians, and nursing staff regarding financial or social concerns.
  • Reports suspected abuse or neglect in accordance with hospital policy.
  • Maintains and updates community resource information and referral networks.
Additional Role Responsibilities (As Applicable)
  • Responds to internal and external referrals and provides appropriate resources and guidance.
  • Participates in utilization review and evaluates appropriateness of social work services.
  • Develops educational and informational materials for patients and staff.
  • Provides in-service education to hospital staff on care coordination and resource management.
  • Participates in appeals related to care coordination denials.
Professional Responsibilities
  • Adheres to all hospital safety policies and procedures.
  • Reports unsafe conditions, incidents, or equipment issues promptly to leadership.

  • Performs other duties as assigned by leadership.

  • Master of Social Work (MSW) or Bachelor's degree in a related field with a strong clinical background.
  • One (1) year of internship experience in social work or one (1) year of experience in case management, utilization review, social services, or a related field.
  • Ability to effectively interact with patients, families, physicians, and interdisciplinary teams.
  • Strong verbal and written communication skills.
Knowledge, Skills, and Abilities – Preferred
  • Three (3) years of experience in case management, social services, or a related field.
  • Certified Case Manager (CCM) or similar certification.
Language Skills
  • Ability to effectively present information and respond to questions from staff, patients, and the public.
  • Ability to read, analyze, and interpret professional literature, technical procedures, and regulations.
  • Report writing is not required for this role.
Mathematical Skills
  • Basic mathematical proficiency.
Reasoning Ability
  • Ability to interpret written, verbal, and diagram-based instructions.
  • Ability to solve practical problems in environments with limited standardization.
