Position: Home Health RN

Location: Lancaster County, PA

Job Type: Contract / Field-Based

Job Overview:

We are seeking an experienced Home Health Registered Nurse (RN) with a minimum of 2 years of experience in home health care. The ideal candidate must have strong expertise in OASIS documentation and wound care including VAC therapy. This role involves providing high-quality patient care in a home setting across Lancaster County.

Key Responsibilities:

• Provide skilled nursing care to patients in their homes

• Complete accurate and timely OASIS documentation

• Perform wound care and VAC (Vacuum-Assisted Closure) therapy

• Monitor patient health and update care plans accordingly

• Educate patients and families on treatment plans and care procedures

• Coordinate with physicians and interdisciplinary teams

• Maintain compliance with healthcare regulations and agency policies

Requirements:

• Active RN license

• Minimum 2 years of Home Health experience

• Mandatory experience with OASIS

• Strong experience in wound care and wound VAC

• Excellent communication and patient care skills

• Ability to work independently in a field setting
