Position: RN Case Manager – Home Health

Location: Plainsboro & West Windsor, NJ (Penn Medicine – Princeton Health)

Job Type: Contract / Field-Based

Job Overview:

We are seeking an experienced Registered Nurse (RN) Case Manager for Penn Medicine Home Health – Princeton Health (South Team). The ideal candidate will have a strong background in home health, case management, and OASIS documentation, with the ability to manage a high-volume patient caseload efficiently.

Key Responsibilities:

• Manage and coordinate care for home health patients
• Perform patient assessments and develop individualized care plans
• Complete accurate and timely OASIS documentation
• Handle a high patient volume of 5–6 visits per day
• Ensure proper documentation and compliance with healthcare standards
• Provide patient and family education on care plans
• Collaborate with physicians and interdisciplinary teams
• Perform wound care (preferred) and monitor patient progress

Requirements:

• Active Registered Nurse (RN) license
• Minimum 2 years of Home Health experience
• Strong Case Management experience (required)
• Mandatory experience with OASIS documentation
• Experience in a high-volume home health setting
• Wound care experience preferred
• Excellent organizational and documentation skills

Work Environment:

• Field-based role covering Plainsboro and West Windsor, NJ
