JOB DESCRIPTION
RN Hospice @ CH Hosp Milwaukee South: ***09
From manager: This nurse will work in the milwaukee territory seeing patients in the field on hospice services. This weekend program (8a-8p) every Saturday and Sunday. This is not a case management position and does not carry a case load. Visit types are daily end of life visits, PRN visits for symptom managment, and occasionally stable routine visits.
● About Facility:
o This is usually Hospice Home Care Remote field work. Locations vary by department/cost center for both IL and WI.
o There is one 18 bed inpatient hospice facility: Aurora Zilber Family Hospice Inpatient Facility, 1155 N. Honey Creek Parkway Wauwatosa, WI 53213
o No hospice facility for field/remote Home Health Aide
● General Information:
o Beds: N/A
o Will you accept a traveler with 1-year experience?
▪ RN: No, 2 years of RN experience, except for Zilber; Zilber will take 1 year RN experience
▪ CNA: 1 year is ok
▪ Home Health Aide: 1 year
o Will you accept a first-time traveler? Yes
● Patient Types:
o Staff will primarily be seeing patients in the community setting, i.e. their home, the home of a family member, Assisted Living Facilities, Group Homes, Nursing Homes, and perhaps a hospice patient in an AAH Hospital.
o Hospice patients/end of life (life expectancy 6 months or less if disease runs its normal course). Common diagnoses are 40% cancer, 60% other medical issues
● Patient Ratios:
o Remote/Field:
▪ RN: Caseload is 13-15, visits per day prorated (5-6 visits per 8 hour shift/day)
▪ CNA: 5-6 visits per day
▪ Home Health Aide: 5-6 visits per day
o Inpatient Hospice:
▪ RN: 1:6-9 patients, dependent on shift.
▪ CNA: 1:6, up to 18 patients at night
▪ Home Health Aide: Does not apply
● Required Certifications:
o RN & CNA: BLS, state license
● Skills required:
o IV skills, chest tube maintenance/pleurex drain, catheter insertion, tube feedings, triage skills, wound care, wound vac, comfortable dealing with PIV, midline, central line, etc., competency with end of life care including pain and symptom management both pharmaceutical and non-pharmaceutical, experience with opioids and opioid equinalagesic table, documentation on EMR platform, independent critical thinker/problem solver but knows when to reach out and ask for help/questions, flexible; inpatient facility does provide palliative sedation
● Support on the Unit only applicable to Aurora Zilber Family Hospice, otherwise it is cost center dependent:
o Receptionist/HUC: Available for all departments; office staff support as well
o Charge nurse: Manager, Team Lead and/or Administrator available
o Phlebotomy? Infrequent; may be asked to perform blood draw. Could be peripheral stick or through a port/line
o Resource Nurse: No, but do have Nurse educator, Hospice Manager, Team Lead, Preceptor
o RT: Resource via our DME division
o Hospitalist: Hospice physician and patient attending physician
● Technology/Equipment:
o EMR: EPIC Hyperspace/ClinDoc or Remote Client, dependent upon role/cost center
o IV pumps:
▪ Aurora Zilber Family Hospice: PCA pump, Alaris IV pump
▪ Wisconsin: Curlin pump
▪ Wisconsin/Illinois: Joey enteral pump
▪ Illinois: Sapphire pump
o Medication dispensing:
▪ Pyxis at Aurora Zilber Family Hospice
▪ Hospice home program: Family/patient/caregiver generally administer, except for IVs/injections managed by RN. We also use a hospice pharmacy benefit manager and pharmacy dispensing/delivery service (courier and/or mail order/ STAT available in IL).
● Orientation:
o Based on needs of individual and discipline: we are committed to ensuring a quality orientation. May be expected to report to Zilber Family Hospice for orientation (1155 N Honey Creek Parkway, Wauwatosa, WI 53213).
o Average for RN is 1 day of equipment pick-up (laptop, PPE, trunk supplies, etc.) and on line training, 2 days EPIC training, and if a visit nurse, they have 3-5 shift of precepted time.

▪ Parking for home team members is per patient place of residence
