JOB DESCRIPTION
RN Hospice Case Manager @ CH Hosp Lake Zurich - Hospice
● Unit Information:
o Hospice team providing care to patients at home and at nursing facilities
o Traveling to Elgin, Bartlett, Carpentersville, and Algonquin.
▪ 103 average census-
▪ 5 patients a day for RN to see. Manage a caseload of 15-20 patients
o Minimum Years of Experience required? 2 years of experience with most recent years in hospice/End of life setting.
o Will you accept a first-time traveler? no
● Patient Types/Common Diagnoses:
o Hospice patients with acute symptom management. Familiar with hospice regulations and supporting families at end of life.
● Patient Ratios:
o RN: 1:5 daily visits with a caseload of 1:20
● Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver's License and/or Insurance:
o RN: BLS and Driver's License with a working car
● Skills required:
o 2 years of hospice experience
o Prior experience with Epic/Remote client required
o Soft skills: Team player, flexible with traveling, adaptable
● Support within the Department: N/A
o Triage nurses
o Team lead – escalations
o Office staff
● Technology/Equipment:
o EMR: Epic Remote client and Rover
o Laptop and cell phone provided
● Floating: If yes, where?
o Yes, may float to other territories to help cover to asap assessments or sick call ins.
● Orientation (facility and unit): 1 week with a preceptor
● Scheduling:
o Weekend rotation: none
