RN Hospice
Shift - 8hr Day Shift (40)
Shift Information - Monday through Friday, with hours from 12pm to 8pm

Unit Information:
- Beds: N/A
- Census: Average patients/day: 2-3, depending on shift
- Minimum Years of Experience required: 2 years of hospice experience
- Will you accept a first-time traveler: Yes
Patient Types/Common Diagnoses:
- Adults
- Heart failure, cancer, other end of life disease processes, etc.
- Will be going into private homes, SNF, assisted living, to do admissions for home hospice program
- BLS, Driver's License, Auto Insurance, Vehicle
- Will be provided laptop, phone
Skills required:
- Explaining hospice care, explaining expectations for when in hospice care, working closely with MDs for orders, communication with Liaisons and Case Manager for hospice, could be doing interventions on initial visit, symptom management for end of life (pain, shortness of breath), comfortable with end-of-life medication and educating providers in how to manage.
- The individual in this role would be responsible for completing two admissions within their shift, or conducting visits if admissions aren't available.
Territory: it will primarily cover the Sheboygan/Manitowoc area, with occasional travel required to Green Bay or Oshkosh
Support within the Department:
- Supervisor/Manager available by phone and chat
- Liaison team available for questions
- Hospice Medical Directors
Technology/Equipment:
- EMR: EPIC – required
- EPIC Hyperspace and EPIC Remote Client used for charting and secure chat
Floating: If yes, where:
- Most common areas will be Burlington and Walworth. Typically, no more than 45 minutes between homes/facilities
Orientation (facility and unit):
- NEO
- 40 hours of unit orientation with staff member
- Weekend rotation: None
- On call: None
- Holiday Expectations: None
- Approval of RTO: to go through manager
- Shift times: Monday – Friday. Open to 8h or 12h shifts. 0800-1630 or 0800-2000. If doing 12h shifts, will have rotating days off each week
Other notes:
- Scrub Color: Business casual or any professional scrubs
- Parking: depending on client home
