Registered Nurse (RN) – Home Care Case Manager

Location: Bala Cynwyd, PA 19004
Contract Duration: 13 Weeks

Schedule: 8:00 AM – 4:30 PM

Requirements:

• Candidates must reside within 30 minutes of the facility
• Must have reliable transportation
• Local candidates strongly required

Job Summary:

We are seeking an experienced and compassionate Registered Nurse (RN) – Home Care Case Manager for a 13-week contract assignment. The RN will be responsible for managing and coordinating patient care in a home care setting, ensuring high-quality, patient-centered services.

Key Responsibilities:

• Perform comprehensive patient assessments in home settings
• Develop and manage individualized care plans
• Coordinate care with physicians and interdisciplinary teams
• Monitor patient progress and adjust care plans as needed
• Educate patients and families on treatment plans and disease management
• Maintain accurate and timely documentation
• Ensure compliance with clinical and regulatory standards
• Provide case management and discharge planning

Required Qualifications:

• Active Registered Nurse (RN) license (PA or compact state)
• Minimum 2 years of RN experience, preferably in home care or case management
• BLS certification from American Heart Association
• Strong clinical, organizational, and communication skills

Preferred Qualifications:

• Home health or case management experience
• Experience with EMR systems
• OASIS documentation experience (preferred)

Work Schedule:

• Day Shift: 8:00 AM – 4:30 PM
