RN - Home Health
Shift: 8hr Day Shift - 40
Shift Information: 8a - 5p
Clinical Interviewing- Yes/No: No - Manager Interview
On Call, Call Back Requirements: No
Weekend Rotation: No unless interested
Holiday Requirements: No unless interested
Floating Requirements: Teammates will travel throughout the below areas: ▪ Terrell, Sherrills Ford, Mount Holly, Maiden, cale, Troutman, McAdenville, Lincolnton, High Shoals, Denver, Dallas, Cramerton, Belmont and cities in between
About Facility:
Home Health Services – serving the following areas: Cleveland, Gaston, Rutherford Cabarrus, Mecklenburg and Rowan Counties
o Radius rule: Traveler's permanent address must be 50 miles outside of home unit
Department Information:
- Typical Caseload:
- Average patients seen/day: (visits are weighted) 6-7 units/day; 30/week
- Average Admissions completed/day or week: Admissions are weighted at 2.40 units – 2 admissions can be expected per day
- Minimum Years of Experience required: 1 year – Home Health experience
- Will you accept a first-time traveler: Yes – if they have had Home Health experience as a nurse
Patient Types/Common Diagnoses:
18 y/o (no peds), wd care, wd vacs, Pleurx drains, cath (foley and SP), feeding tubes, IVs (peripheral and PICCs/Ports), DM, HF, med and disease mgmt.
Teammates will travel throughout the below areas:
▪ Terrell, Sherrills Ford, Mount Holly, Maiden, cale, Troutman, McAdenville, Lincolnton, High Shoals, Denver, Dallas, Cramerton, Belmont and cities in between
Required License:
- National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver's License and/or Insurance:
- Driver's License and proof of Insurance
- BLS
Skills required:
- Charting experience with Homecare Homebase is required (no exceptions), Oasis, hospice philosophy, infection control practices, wound vacs, wound care, PICC line dressing changes, port access, IV infusions, drains, critical thinking skills, catheter insertion, drains
- Ability to drive at night
Support within the Department:
- Leadership
- LPN/CNA
- Peers
- Case Management
Technology/Equipment:
- EMR: HCHB (Homecare Homebase)
- Samsung note 9 phones
- EPIC Rover – view only
- Focura - will upload discharge instructions and other important docs such as wound pictures
- NEO
- 2 days departmental
Scheduling:
- Weekdays M-F 8a-5p
- RTO: Manager approval
Other notes:
- Scrub Color: Navy
- Do they schedule their own appointments with this position: - they will schedule the visit with the patients the day prior to the visit planned.
- Do they still get paid for cancelled appointments: cancelled appts is not counted as visit equivalents. They are required 30 VE a week
- Will a company car be available: No
- Is a company cell phone provided: A phone is provided and is used for documentation and calling patients/providers.
