Position Details

Responsibilities
- Evaluate and develop individualized treatment plans for patients in a home health setting
- Provide therapeutic exercises and mobility training
- Educate patients and families on treatment plans and recovery goals
- Maintain accurate documentation and compliance with agency standards
- Collaborate with interdisciplinary healthcare teams to ensure quality patient care
Requirements
- Active Physical Therapist license
- Minimum 2 years of recent PT experience
- Home Health experience preferred
- Valid driver's license and reliable transportation required
- Strong communication and documentation skills
Additional Information
- Candidate should be comfortable with daily travel within Lancaster County
- Mileage/travel expectations will be discussed during the interview process
- Local candidates preferred, but travelers may be considered depending on availability and distance from coverage area
