Position Overview

We are seeking an experienced and compassionate Licensed Practical Nurse (LPN) with a strong background in Home Care to join our healthcare team. The ideal candidate will have at least 2 years of home health experience and must be highly proficient in wound care management.

This role involves providing skilled nursing care to patients in their homes while ensuring high-quality clinical support, patient safety, and continuity of care. Candidates must be comfortable traveling within the assigned coverage areas and working independently in a home care environment.

Key Responsibilities

• Provide skilled nursing care to patients in home care settings
• Perform and assist with wound care treatments, dressing changes, and skin assessments
• Monitor patient conditions and report changes to the supervising RN or physician
• Administer medications and treatments as prescribed
• Assess patient needs and support individualized care plans
• Document patient care accurately in EMR systems
• Educate patients and family members on treatment plans and home care instructions
• Maintain infection control and patient safety standards
• Coordinate care with interdisciplinary healthcare teams
• Travel between assigned patient homes within designated zip codes

Requirements

• Active and unencumbered LPN License
• Minimum 2 years of Home Care experience required
• Must be proficient in Wound Care to be considered
• Strong clinical assessment and patient care skills
• Excellent communication and documentation abilities
• Reliable transportation required for patient visits
• Ability to work independently in field-based settings

Preferred Qualifications

• Previous experience in skilled home health or community nursing
• Experience with EMR documentation systems
• Strong time management and organizational skills
