About Facility:

--this opening will be located in the NORTHERN area of Green Bay/Oshkosh/Manitowoc/Sheboygan/Grafton - may be 1 day assignment of Green Bay/Oshkosh, another day Manitowoc/Sheboygan, etc. Would not cover full territory in same day.

Unit Information:

o Beds: N/A

▪ Census? Average patients/day? 2-3 patients per day

o Minimum Years of Experience required? 2 years. Would prefer hospice liaison experience, but open to other hospice experience

o Will you accept a first-time traveler? Yes

o Will monitor queue and consult with patients that are referrals from hospitals or SNF for hospice

o If patients are transitioned to hospice in a hospital setting, the liaisons will provide assessments and recommendations on the proper hospice setting for a patient. Liaisons can also admit to hospice services in the hospital setting

o Could travel to Aurora Sinai, Aurora St. Luke's, Aurora West Allis, Aurora Grafton, Aurora Hartford, Aurora St. Luke's South Shore, and Aurora Summit Hospitals. Will be assigned 1-2 hospitals, depending on hospital size and census

Patient Types/Common Diagnoses:

o Adults

o Heart failure, cancer, other end of life disease processes, etc.

Required License, National Certification, Certs:

o BLS, Driver's License, Auto Insurance, Vehicle

o Will be provided laptop, phone

Skills required:

o Symptom management for end of life (pain, shortness of breath), comfortable with end of life medication and educating providers in how to manage, comfortable speaking and collaborating with physicians, comfortable providing education on hospice benefits and hospice philosophy, understanding the different levels of hospice care (general inpatient, routine, respite), understanding different levels of service that hospice can be provided

o EPIC experience required

Support within the Department:

o Supervisor/Manager available by phone and chat

o Liaison team available for questions

o Hospice Medical Directors

Technology/Equipment:

o EMR: EPIC – required

▪ EPIC Hyperspace and EPIC Remote Client used for charting and secure chat

Orientation (facility and unit) :

o NEO

o 40 hours of unit orientation with staff member

Other notes:

o Scrub Color: Business casual or any professional scrubs; white coat preferrable but not required

o Parking: Free
