Position Summary:

We are seeking a compassionate and dedicated Physical Therapist to provide high-quality patient care in a home health setting. The Physical Therapist will evaluate patients, develop individualized treatment plans, and help patients improve mobility, strength, balance, and overall functional independence. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a patient-centered approach to care.

Key Responsibilities:

• Perform initial and ongoing physical therapy evaluations for patients in home health settings.
• Develop and implement individualized treatment plans based on patient needs and physician orders.
• Provide therapeutic exercises, gait training, balance training, and mobility assistance.
• Educate patients and family members on treatment plans, exercises, safety precautions, and home care techniques.
• Monitor patient progress and modify treatment plans as needed.
• Maintain accurate and timely clinical documentation in accordance with agency and regulatory standards.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals to coordinate patient care.
• Ensure compliance with infection control, safety policies, and HIPAA regulations.
• Participate in interdisciplinary team meetings and case conferences.

Qualifications:

• Graduate of an accredited Physical Therapy program.
• Active Physical Therapist (PT) license in the state of Pennsylvania.
• Current BLS/CPR certification required.
• Home health experience preferred.
• Strong assessment, communication, and clinical documentation skills.
• Ability to work independently and manage patient schedules efficiently.
• Reliable transportation and valid driver's license required.

Preferred Skills:

• Experience with geriatric and rehabilitation patient populations.
• Knowledge of home health regulations and documentation standards.
• Strong interpersonal and patient education skills.
