General Information:
o Approximately how many visits per day- 6 total points per day
o Minimum Years of Experience required- 1 year
o Will you accept a first-time traveler- Yes, if seasoned to home health
o Home Health Therapy Services
o Radius rule: Traveler s permanent address must be 50 miles outside of home unit
Patient Types/Common Diagnoses:
o Adults – only
o Chronic diseases, wound care, IV, pleurx, wound vac
Required Licensure & Certifications:
o Driver's License and proof of Insurance required for all positions
o PT: BLS; State Licensure required. Minimum Education Requirement: BA Degree
o OT: BLS; State Licensure required. Minimum Education Requirement: BA Degree
Skills required:
o Charting experience with Homecare Homebase is required (no exceptions), OASIS-E (assessment tool) experience
o IV, pleurx, wound vac, wound care, assessment, disease teaching
o Experience with geriatric/Adult, chronic disease management, maintenance notes
Support within the Department:
o Departmental Leadership
Technology/Equipment:
o Samsung note 9 phones
o EMR: Homecare Homebase - required
Floating:
o N/A – Home health
o This branch sees York and Lancaster counties (South Carolina); could be asked to float into Union or Mecklenburg County but not likely
Weekend-only position could drive up to 100 miles, depending on case load
Orientation (facility and unit):
o NEO
o 5 days orientation
