JOB DESCRIPTION
About Facility:
Home Health Services – York County and Lancaster County
Home Office Address: 226 Northpark Dr., Rock Hill, SC, 29730
General Information:
Approximately how many visits per day? 6 total points per day, no less than 30 points per week
Minimum Years of Experience required? 1 year of HH experience
Will you accept a first-time traveler? Yes, as long as they have HH experience
Radius rule: Traveler s permanent address must be 50 miles outside of home unit
Patient Types/Common Diagnoses:
Adults only
Chronic diseases, wound care, IV, Pleurx, wound vac, post-stroke, post-rehab, post-joint replacements for hip/knee/shoulder
Required Licensure & Certifications:
Driver's License and proof of Insurance required for all positions
PTA: BLS, State license required. Minimum Education Requirement: Associate or above
Skills required:
For all: Charting experience with Homecare Homebase is required (no exceptions)
PT/OT (no PTA): OASIS-E (assessment tool) experience
Experience with above patient types, disease teaching, experience with geriatric/Adult, chronic disease management, maintenance notes, gait speed and use of gait belt
Support within the Department:
Departmental Leadership
PT will oversee PTAs
Technology/Equipment:
Samsung note 9 phones, laptop, and other equipment
EMR: Homecare Homebase - required
Floating:
Weekend-only position could drive up to 100 miles, depending on case load
N/A – Home health
This branch sees York and Lancaster counties (South Carolina); could be asked to float into Union or Mecklenburg County but not likely
Orientation (facility and unit):
NEO
5 days orientation
Scheduling:
Weekend rotation: no weekends, unless posted as a weekend only position (Fri/Sat/Sun)
No call
Holiday Expectations: if would like to work holidays, would be allowed
RTO requests: Manager approval
