Position Overview

Penn Medicine Home Health Princeton Health is seeking an experienced Registered Nurse (RN) Case Manager to join their South Team serving patients throughout Plainsboro and West Windsor, NJ. The ideal candidate will have recent home health case management experience, strong OASIS proficiency, and the ability to manage a high-volume patient caseload while providing exceptional patient-centered care in the home setting.

Responsibilities

• Manage and coordinate comprehensive home health care services for assigned patients.
• Conduct initial assessments, recertifications, resumptions of care, and discharge visits.
• Complete accurate and timely OASIS documentation in accordance with Medicare guidelines.
• Develop, implement, and revise individualized care plans based on patient needs.
• Coordinate care with physicians, therapists, social workers, and interdisciplinary healthcare teams.
• Provide patient and family education regarding disease management, medications, treatments, and home safety.
• Perform skilled nursing assessments and interventions in the patient's home.
• Monitor patient progress and communicate changes in condition to appropriate providers.
• Maintain accurate documentation and meet productivity expectations in a high-volume home health environment.
• Ensure compliance with agency policies, state regulations, and CMS requirements.

Requirements

• Active Registered Nurse (RN) License.
• Minimum 2 years of recent Home Health RN experience required.
• Recent Case Management experience required.
• Strong OASIS documentation experience required.
• Experience managing a high-volume home health caseload.
• Ability to independently prioritize and coordinate patient care.
• Valid Driver's License, reliable transportation, and auto insurance.
• Current BLS Certification.

Preferred Qualifications

• Wound Care experience strongly preferred.
• Experience with Medicare-certified home health agencies.
• Strong clinical assessment and care coordination skills.
• Experience seeing 5–6 patients per day while maintaining timely documentation.

• Strong knowledge of OASIS, care planning, and home health regulations.
• Excellent communication, organization, and time-management skills.
