Assignment Details
- Facility: Smyrna Health & Wellness Center – Smyrna, DE
- Position: Physical Therapist – Home Health (Travel)
- Duration: 13 Weeks
- Start Date: ASAP (Scheduled Orientation)
- Schedule: Monday – Friday, 8-hour day shifts
- Guaranteed Weekly Hours: 40
- Pay Cycle: Weekly Pay
- Coverage Area: Sussex County and Southern Delaware (within approximately a 60-mile radius of Smyrna).
- Primary Service Regions:
- Georgetown Region: Georgetown, Harbeson, Ellendale, and Millsboro.
- Lewes/Rehoboth Beach Region: Lewes, North Shores, and Rehoboth Beach.
- Required Experience: Home Health experience with proficiency in OASIS documentation.
- License Required: Active Delaware Physical Therapist License.
- On-Call/Weekend Requirements: To be discussed with the facility.
Credentialing Requirements
- Allow approximately 5 weeks for credentialing.
- Non-Delaware residents must travel to Delaware after acceptance for in-person fingerprinting.
- Fingerprint results generally require 3–4 weeks to process before credentialing is complete.
- Provide skilled physical therapy services to adult and geriatric patients in a home health setting throughout Sussex County and Southern Delaware.
- Perform comprehensive initial evaluations, reassessments, and develop individualized treatment plans based on physician referrals and patient goals.
- Complete OASIS assessments (Start of Care, Recertification, Discharge, and Follow-up) accurately and in compliance with Medicare and agency standards.
- Deliver evidence-based interventions including therapeutic exercise, gait training, balance training, transfer training, strengthening, neuromuscular re-education, and fall prevention.
- Evaluate patient mobility, strength, endurance, pain levels, and functional limitations to establish measurable rehabilitation goals.
- Educate patients and caregivers on home exercise programs, assistive device use, injury prevention, energy conservation, and strategies to maximize independence.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, social workers, and case managers to coordinate comprehensive patient care.
- Monitor patient progress, modify treatment plans as appropriate, and communicate significant clinical changes to the interdisciplinary healthcare team.
- Maintain timely and accurate electronic documentation while meeting productivity expectations within an 8-hour workday.
- Manage a daily caseload consisting of approximately 6 revisits (typically including one evaluation) or 4–5 revisits plus one admission.
- Travel independently between patient homes while efficiently managing scheduling within the assigned geographic territory.
- Primarily provide care within one designated service region, with occasional coverage in neighboring areas based on operational needs.
- Participate in interdisciplinary case conferences, discharge planning, and quality improvement initiatives.
- Ensure compliance with Medicare regulations, home health documentation standards, agency policies, HIPAA requirements, and infection prevention guidelines.
- Deliver compassionate, patient-centered care focused on improving functional mobility, reducing hospital readmissions, and enhancing patients' quality of life.
