Duration: 13 Weeks
Location: 1650 Champlin Avenue, Utica, NY 13502
Unit: Long-Term Care (LTC)
Shift: 2:30 PM – 10:30 PM
Schedule: 5 × 7.5-hour shifts | 37.5 hours/week
Guaranteed Hours: 37.5 hours/week
Weekend Requirement: Every Other Weekend (EOW)
Block Scheduling: Not guaranteed or promised
Local Candidates: Accepted – Local rate applies to candidates residing within 75 miles

Position Requirements

Minimum 1 year of recent LTC experience required.

Travel experience must be clearly documented on the resume.

Active, unencumbered New York LPN license required.

Current BLS certification required.

NURSYS license verification report required, showing active NY LPN license and all active licenses.

Skills checklist must match the LPN/LTC specialty.

Valid driver's license with an address matching the candidate's demographic information.

RTO (Requested Time Off) must be submitted at the time of submission.

Up-to-date resume required.
