Shift: 12hr Day Shift (24) Shift Information: 8a-8p Saturdays and Sundays Clinical Interviewing- No On Call, Call Back Requirements: None Weekend Rotation: Every weekend Holiday Requirements: Holidays that fall on the weekend Floating Requirements: May need to assist with wider coverage if there is a call in at a local territory

RN Hospice HH -Nursing: SCT (Supportive Care Team): ***41

10 week contract servicing the Racine Kenosha territory. Includes but not limited to: Racine, Kenosha, and Mt Pleasant.
Shifts: Saturday and Sunday, 8a-8p, 24-hr/week contract

About Facility:

• This is a Home Hospice position so will go into patient s homes.
• See below for territory coverage, will depend on requisition as to what territory

Unit Information:

Minimum Years of Experience required: 2 years, Home Health or Home Hospice experience

Will you accept a first-time traveler: Yes, if they have case management experience in the home setting.

Patient Types/Common Diagnoses:

• Failure to thrive, advanced cancers, end stage COPD, end of life, terminally ill, end stage disease processes
• Attend after hour visits that are called into our triage line (PRN visits - symptom management, fall, etc., death visits). Some issues can be resolved via phone and do not require an in-person visit

Patient Ratios: RN: 0-6, depending on call-ins for the night

Required License, National Certification, Certs:

• BLS, Active Driver s License, State of WI RN License, own vehicle, auto insurance
• Will be provided badge, laptop, cell phone, and trunk kit once on site

Skills required:

• Comfortable performing focused assessments and critically thinking, comfortable investigating patient agitation issues (when s the last time they urinated: Is their foley crimped: Etc.), exchanging foley, troubleshooting feeding tubes, wound care, post-fall assessment, comfortable with death visits and conversing with medical examiner or coroner, comfortable administering narcotic pain medications (typically patients will have mediation kit at home), identifying potential drug diversions in the home

Support within the Department:

• Lead RN 24/7 to call if any issues. Will update Lead RN after visits and give report
• Other Hospice SCT members available via phone

Technology/Equipment:

• EMR: Epic (Rover), Remote Client
• IV Pump: Sapphire (very rare to see in a home)

Floating: If yes, where:

• May occasionally float slightly outside of their assigned territory if a call comes in that they are closer to and that territory nurse is in another call. Never over an hour away from home territory.

Orientation (facility and unit) :

• NEO at territory office
• 3 shifts (1 week) with an SCT nurse

Scheduling:

• Weekend rotation: Every weekend position
• On call: None
• Holiday Expectations: 1 summer/1 winter
• Approval of RTO: to go through manager
