REQUIREMENTS:

OT LICENSURE in the State of Texas,
BLS (AHA OR ARC)
2 years with concentration in NICU required.

ANY SOLID COLOR SCRUBS
PATIENT RATIO - ONE OT AND ONE ST IN NICU - ALL PATIENTS ON OT CASELOAD SEEN 3X5 TIMES PER WEEK
NUMBER OF BEDS - 21
EMR - MEDHOST
TRAVELERS SUBMITTED WITHOUT THE NECESSARY INFORMATION OR REQUIREMENTS WILL BE REJECTED
MUST INCLUDE PERMANENT HOME ADDRESS AND SS# ON THE SUBMISSION
TRAVELERS MUST BE GREATER THAN 50 MILES FROM FACILITY
