Physical Therapist – Home Health

Job Summary

We are seeking a qualified Physical Therapist (PT) to provide skilled home health physical therapy services to patients throughout the North Princeton, NJ coverage territory.

The Physical Therapist will provide comprehensive patient care in the home setting, including Start of Care (SOC) visits and revisit/recertification visits, while ensuring accurate clinical documentation and compliance with home health standards.

Key Responsibilities

• Provide skilled physical therapy services to patients in their homes.
• Conduct comprehensive assessments and develop individualized treatment plans based on patient needs.
• Perform Start of Care (SOC) visits and revisit visits.
• Evaluate patients' functional abilities, mobility, strength, balance, and safety needs.
• Develop and implement appropriate therapeutic interventions to improve mobility, independence, and quality of life.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Educate patients and caregivers on exercises, mobility techniques, home safety, and disease management.
• Complete required OASIS documentation accurately and within established timeframes.
• Document patient visits, assessments, interventions, and progress in EPIC.
• Coordinate patient care with nurses, physicians, occupational therapists, social workers, and other members of the interdisciplinary team.
• Communicate changes in patient condition to the appropriate healthcare providers.
• Ensure compliance with Medicare, regulatory, and agency requirements for home health services.
• Maintain timely and accurate clinical documentation for all patient encounters.

Required Qualifications

• Current and valid Physical Therapist (PT) license in New Jersey.
• Recent and relevant Home Health Physical Therapy experience required.
• Previous experience performing Start of Care (SOC) and revisit visits.
• Hands-on experience with OASIS required.
• EPIC experience required.
• Strong clinical assessment, documentation, communication, and time-management skills.
• Ability to travel throughout the North Princeton, NJ coverage territory.
• Ability to work independently while coordinating effectively with the interdisciplinary care team.

Preferred Qualifications

• Experience working with Medicare home health patients.
• Familiarity with home health documentation and regulatory requirements.
• Experience managing a diverse home health caseload.
