Description

Physical Therapist - Windsor El Camino Car - 1st Shift (08:00 to 16:30)

Position Summary:
The Physical Therapist provides outpatient rehabilitation services primarily in an in-home setting for the older adult population. This setting may include a patient's private residence or a room in an independent living facility. The Physical Therapist assesses, develops, and delivers individualized physical therapy programs to facilitate rehabilitation, restore, reinforce, and enhance patient performance. They also instruct and train patients and caregivers to promote independence and productivity while addressing pathology and promoting health.

Core Responsibilities:

  • Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel as applicable and in accordance with state licensure requirements and professional standards.
  • Adhere to department and patient treatment schedules, ensuring timely clinical and billing documentation and participation in work-related meetings.
  • Attend and contribute to patient care and staffing conferences and other related meetings.
  • Make referrals to other disciplines and agencies to address the comprehensive needs of patients.
  • Consult with rehabilitation team members to select appropriate therapeutic regimens consistent with patient needs and capabilities.
  • Adapt treatment programs and plans of care according to individual patient needs.
  • Promote a safe environment by enforcing the elimination of fire and safety hazards.
  • Order, clean, maintain, and report the need for repair of supplies and equipment as necessary.
  • Ensure patients and families receive high-quality, compassionate service that recognizes individual needs and rights.
  • Screen, examine, and evaluate patients through history, systems review, and appropriate tests to establish diagnoses, identify impairments, predict improvement levels and timeframes, identify precautions/contraindications, and design plans of care.
  • Develop treatment goals and methods collaboratively with patients and caregivers, implement treatment plans, and complete all related documentation.
  • Apply therapeutic techniques including but not limited to therapeutic exercise, functional training, manual therapy, airway clearance, integumentary repair, electrotherapeutic and physical agent modalities, health and wellness education, chronic disease management, falls assessment and intervention, and case management.
  • Design, fabricate, fit, and apply adaptive devices including orthotics and prosthetics, and train patients in their use.
  • Select activities suited to individual physical and mental capacity, intelligence level, and interest.
  • Instruct patients in mobility skills and techniques, evaluating progress, attitudes, and behavior related to rehabilitative potential.
  • Perform routine reexaminations to modify or progress treatment plans as needed.
  • Incorporate health and wellness programming into treatment plans.
  • Perform home and community mobility assessments and provide recommendations for safe mobility.
  • Perform other related duties as required.

Qualifications:

  • Valid Physical Therapy license in the state(s) of practice or proof of license eligibility.
  • Master's degree in Physical Therapy; or Master's degree in a Physical Therapy related field plus a Bachelor's degree in Physical Therapy; or Bachelor's degree in Physical Therapy with 5 years of progressively responsible experience.
  • Foreign Physical Therapy degrees must be evaluated as equivalent to U.S. degrees.
  • Valid driver's license or reliable, independent transportation to and from patient residences.
  • Good organizational and time management skills.
  • Strong verbal and written communication skills.

Compliance:

Complies with applicable legal requirements, standards, policies and procedures including but not limited to those within the Compliance and Ethics Program, Standard/Code of Conduct, Federal False Claims Act and HIPAA.
Participates in required orientation and training programs.
Promptly reports concerns and suspected incidences of non-compliance to supervisor, Compliance Liaison or to the Compliance Officer via the Integrity Hotline.
Cooperates with monitoring and audit functions and investigations.
Participates, as requested, in quality assurance and process improvement activities.
