Licensed Practical Nurse (LPN) – Long-Term Care

⏰ Schedule: Full-Time – 40 Hours/Week (Day/Evening Rotation)

Position Overview

We are seeking an experienced Licensed Practical Nurse (LPN) to join a long-term-care healthcare team in New York. The ideal candidate will have at least 1 year of LPN or relevant long-term-care experience, a current New York LPN license, and strong patient/resident care skills.

This position involves providing direct and indirect resident care under the direction of licensed nursing personnel and in accordance with established care plans, facility policies, and scope-of-practice requirements.

Required Qualifications

• Active New York LPN License – Required
• BLS through AHA or RQI – Required
• Minimum 1 year of LPN experience
• Long-term-care (LTC) or comparable healthcare experience preferred
• High School Diploma or GED – Required
• Ability to provide safe, compassionate care to adult and geriatric residents
• Strong communication, documentation, and teamwork skills
• Holiday availability is required

Key Responsibilities

• Provide and support resident care according to established plans of care, facility policies, and applicable scope-of-practice requirements.
• Assist with residents' activities of daily living, including hygiene, grooming, dressing, toileting, incontinence/catheter care, positioning, and skin care.
• Monitor resident safety and maintain a clean, comfortable, and secure care environment.
• Respond promptly to call lights and alarms and follow established fall-prevention and safety procedures.
• Support residents with nutritional needs, including meal delivery, feeding assistance, adaptive equipment, and nourishment as applicable.
• Assist with mobility, transfers, ambulation, range-of-motion activities, and appropriate use of assistive devices.
• Respect resident preferences, choices, rights, and refusals.
• Observe residents for changes in physical, emotional, and psychosocial condition and promptly communicate concerns to the Charge/Supervising Nurse.
• Maintain accurate and timely documentation in accordance with facility protocols.
• Follow infection-control, safety, privacy, and regulatory requirements.
• Maintain awareness of potential occupational exposure to blood, body fluids, and tissues and follow appropriate precautions.
