Contract Duration: 14 Weeks
Work Schedule: Monday–Friday, 8:30 AM–5:00 PM
Work Setting: Home Health / Hospice – Home Visits Required
Candidate Requirement: Local Orange County (OC) candidates only, with a primary focus on South Orange County.

Job Summary

The Clinical Nurse I – Home Health/Hospice RN provides direct patient care to assigned patients in home health and/or hospice settings in accordance with the California Nurse Practice Act and established clinical standards.

The RN collaborates with an interdisciplinary healthcare team to ensure the delivery of safe, timely, and high-quality patient care. Responsibilities include patient assessments, nursing interventions, medication administration, pain management, infusion therapy, specimen collection, nutritional support, patient and family education, and discharge planning.

The nurse utilizes the nursing process and evidence-based practices to develop, implement, evaluate, and adjust individualized care plans. The RN also consults appropriate clinical resources when addressing complex medical, ethical, or legal issues and performs other duties as assigned.

Key Responsibilities

• Provide direct nursing care to patients in home health and/or hospice settings.

• Conduct comprehensive physical assessments and monitor patient conditions.

• Perform nursing interventions, including IV therapy, infusion services, and medication administration.

• Provide effective pain management and symptom control.

• Collect specimens and support patients' nutritional needs.

• Educate patients and families regarding treatment plans, medications, and ongoing care.

• Assist with discharge planning and continuity of care.

• Collaborate with interdisciplinary healthcare professionals to coordinate patient care.

• Apply evidence-based nursing practices and evaluate the effectiveness of care plans.

• Consult appropriate clinical personnel when addressing complex patient care concerns.

• Perform additional duties as assigned.

Required Qualifications

Home Health RN:

• Minimum of two (2) years of acute clinical nursing experience.

• Previous clinical home health nursing experience is required.

Hospice RN:

• Minimum of two (2) years of acute clinical nursing experience or home health nursing experience.

Preferred Qualifications

• Previous hospice nursing experience is preferred.

Important Assignment Details

• This is a local assignment; Orange County candidates only.

• Preference is for candidates located in or able to cover South Orange County.

• Home visits are an essential part of the role.

• Candidates must be available to work Monday through Friday, 8:30 AM–5:00 PM, for the duration of the 14-week contract.
