Travel Long Term Care (LTC) Physical Therapist

This position is for a Physical Therapist working day shifts from 8:00 AM to 4:30 PM in a Skilled Nursing Facility (SNF) with a guaranteed 35 hours per week. The Physical Therapist will provide rehabilitation services to residents recovering from injury, illness, surgery, or functional decline. The role focuses on improving mobility, strength, balance, and independence in daily activities. The therapist will work closely with nursing staff and rehabilitation teams to develop treatment strategies that support recovery and improve overall quality of life for residents in a long-term care or post-acute rehabilitation setting.

Requirements:
Candidates must hold an active Physical Therapist license in the appropriate state and have graduated from an accredited Physical Therapy program. Previous experience in a Skilled Nursing Facility, rehabilitation center, or post-acute care environment is preferred. Applicants should demonstrate knowledge of therapeutic exercise programs, gait training, balance rehabilitation, and fall prevention strategies. Strong communication and documentation skills are required to collaborate with interdisciplinary healthcare teams and maintain accurate patient records within electronic medical record systems.
