Physical Therapist assignment in a Skilled Nursing Facility setting for a 13-week contract. This role offers a consistent day shift schedule from 8:00 AM to 4:30 PM with 35 guaranteed hours per week. The position is ideal for a therapist who is comfortable working with patients requiring rehabilitation, mobility support, and restorative care in a post-acute environment. The therapist will be expected to provide quality patient care, support functional improvement, and contribute to overall recovery goals throughout the duration of the contract.

Requirements:
Candidates should have experience working as a Physical Therapist in a Skilled Nursing Facility or similar rehabilitative care setting. The clinician must be able to manage a full caseload, provide individualized treatment plans, and work collaboratively with interdisciplinary teams to support patient progress. Strong communication, documentation, and patient mobility training skills are important for success in this role. All required licensure, certifications, and compliance documents must be active and up to date before the start of the assignment.
