Description

ake a difference in a skilled nursing setting as a Physical Therapist (PT) for a 13-week assignment working 8:00 AM – 4:30 PM with 35 guaranteed hours per week. The Physical Therapist will evaluate, treat, and develop individualized rehabilitation plans for residents recovering from illness, injury, surgery, or functional decline. This role focuses on improving mobility, strength, balance, safety, and overall quality of life while collaborating with interdisciplinary healthcare teams. Requirements: Minimum 1 year of Physical Therapy experience required
LTC/SNF or Rehab experience required
Active State Physical Therapist License required
BLS Certification required
Strong patient assessment and treatment planning skills required
Experience with geriatric and rehabilitation populations preferred
Ability to work independently and as part of a multidisciplinary team required
Excellent communication and documentation skills required
