Active Hawaii Physical Therapist (PT) license required and must be provided with submission 
Minimum 2 years of Physical Therapist experience required 
Minimum 1 year of Home Health Physical Therapy experience required 
Bachelor’s degree in Physical Therapy required; Master’s degree preferred 
Current AHA BLS and CPR certification required 
Epic experience required 
Previous travel experience preferred 
Strong skills in patient evaluation, individualized treatment planning, therapeutic interventions, discharge planning, and clinical documentation 
Ability to meet productivity expectations of 25 points per week 
Must have reliable personal transportation, a valid driver’s license, and current auto insurance for home health visits 
Ability to travel between assigned patient locations throughout Oahu 
Strong communication, independent case management, and interdisciplinary collaboration skills
