Job Summary:
Physical Therapist Home Health (PT HH) position located in a designated area, with a 12-week assignment. The role involves providing skilled therapy services in a home health setting, with schedules typically from 8:00 AM to 4:00 PM or 9:00 AM to 5:00 PM, and as needed on weekends. The assignment supports experienced professionals committed to delivering exceptional patient care.

Job Details:

• Schedule: Dayshift, 8 hours per day, minimum of 40 hours per week.
• Required Qualifications: Licensure as a Physical Therapist (PT), minimum of 1 year of relevant experience, ability to work in a home health environment, and proximity to the assignment location.
• Preferred Qualifications: Experience with Epic electronic health record system is preferred but not required.
• Additional Information: Assignment duration is 12 weeks, starting on 08/31/2026 and ending on 11/22/2026. Flexibility to work weekends as needed is required.
