Job Summary:
A Physical Therapist specializing in Home Health located in Albemarle, North Carolina. This position involves providing skilled therapy services to adult patients with various diagnoses, including chronic diseases, orthopedics, and post-surgical care. Candidates are required to bring their state licensure on the first day of work.

Job Details:

• Location: Albemarle, North Carolina
• Schedule: Days shift, minimum of 40 hours per week
• Required Qualifications: Valid drivers license with proof of insurance, state licensure as a Physical Therapist, Basic Life Support (BLS) certification, at least 1 year of Home Health experience, experience with geriatric and pediatric patients, chronic disease management, maintenance of OASIS-E documentation, Homecare Homebase Electronic Medical Record (EMR) experience
• Additional Information: The therapist will schedule visits with patients the day prior to the planned visit. Cancelled appointments are not counted as visit equivalents; a minimum of 30 visit equivalents per week is required. No company car will be provided. A phone will be supplied for documentation and patient/provider communication. No weekend rotation, no call, and no major holidays. RTO requires manager approval. The schedule cycle is every 13 weeks. Department culture emphasizes a team environment. Scrub color is gray; parking is free.
