Job Summary:
Home Health Physical Therapist position based in San Ramon, California. This role involves providing outpatient therapy services to patients within designated service areas, with an emphasis on patient-centered care and clinical excellence. The position requires flexibility to travel outside the primary service area and work weekends as needed to meet patient needs.

Job Details:

• Schedule: Days shift, 10 hours per shift, with a minimum of 40 hours per week.
• Required Qualifications: Current California Physical Therapist (PT) license, BLS certification, valid drivers license, auto insurance, auto registration, and compliance with TB, MMR, Varicella, HepB, TDaP (10 years), Flu (annual), drug screening, and physical examination requirements.
• Specialty: Physical Therapist Home Health (PT HH).
• Experience: 1 year of clinical experience required.
• Start Date: 08/24/2026.
• Travel and Coverage: Willingness to travel outside designated service areas and work weekends may be required. Additionally, flexibility to see COVID-positive and COVID PUI (Person Under Investigation) patients.
• Patient Visit Payment: Compensation is based on per visit basis, which encompasses drive/travel time, visit time, documentation, and communication with patients and healthcare providers.
• Additional Information: The agencys contract must mirror the confirmation details, including start and end dates. All Requested Time Off (RTO) must be disclosed at the time of submission; post-acceptance RTO requests are not permitted.
