Job Summary:
A Physical Therapist specializing in Home Health services is needed in Portland, Oregon. This position involves providing outpatient therapy to adult and geriatric patients in a home health setting. Candidates are required to bring their license on the first day of assignment. The position starts on 07/13/2026.

Location: Portland, Oregon

Schedule: Day shifts, 8 hours, minimum 40 hours per week
Patient Ratio Experience: Up to 6 patients per day with a 1:1 ratio, depending on productivity
Travel: Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day; traveler is expected to have their own transportation.
Additional Information: Traveler will use a charting system including EPIC and Rover applications. Facility requires primary series COVID vaccination in addition to one booster.
Clinical Skills & Qualifications: Preferred experience with home health OASIS documentation, lifting and transfer orders, and scheduling appointments.
