Job Summary:
The organization is seeking a qualified Physical Therapist specializing in Home Health in the North Dakota area. The role involves providing patient care within a 60-mile radius of Valley City, ND, focusing on patients recovering from surgeries, falls, and other conditions. The position requires dedication to delivering high-quality physical therapy services in a home health setting, adhering to established clinical standards and operational protocols.

Job Details:

• Schedule: Monday - Friday, 8:00 AM - 5:00 PM, Days shift, with a minimum of 40 hours per week.
• Required Qualifications: Bachelor of Science in Nursing (BSN) not required; ND Physical Therapy License; BLS certification; minimum 1 year of experience; must have PT related skills and previous home health experience; valid Drivers License and Auto Insurance.
• Preferred Qualifications: Prior home health experience and familiarity with HomeCare/HomeBase electronic health records system.
• Responsibilities: Treat 5-7 patients daily, managing conditions such as surgeries, falls, and other common diagnoses for home health and hospice patients.
• Additional Information: Candidates must possess a current Driver's License and Auto Insurance. No clinical involvement or manager interview required.
• Restrictions: Candidates may not have been full-time, part-time, or PRN with any facility associated with CommonSpirit, including CHI and Dignity, within the past 12 months. RTO approval required upfront; no additional RTO will be granted. The organization can cancel up to four shifts per 13-week assignment, or three shifts for shorter assignments.
• Charting System: Other computerized systems not listed.
• Dress Code: CSH color scrub or business casual.
