Description
Job Summary:
The organization is seeking a qualified Physical Therapist (Home Health) for a contract assignment in Durham, North Carolina. This position involves providing skilled physical therapy services in both patient homes and care facilities, following physicians' orders and the plan of care. Candidates must bring their license on the first day and have relevant experience and certifications.
Job Details:
- Location: Durham, North Carolina, 1005 Slater Road, Suite 105, with primary coverage in Person County and overflow to N. Durham.
- Shift: Monday through Friday, 8:00 AM to 5:00 PM, with a minimum of 40 hours per week.
- Duration: 13 weeks, with possible hire at the end of the contract.
- Responsibilities: Provide physical therapy services in accordance with physicians' orders, evaluate patients' levels of function, update treatment plans, observe and document patient conditions, instruct family and agency personnel, assess the home environment, and participate in in-service education.
- Supervision: Provide on-site supervision for up to 2 full-time equivalent Physical Therapy Assistants (PTAs), perform assessments and adjustments at least once a month, and communicate regularly with supervising physical therapists.
- Qualifications:
- Current and unencumbered license to practice physical therapy in North Carolina.
- Successful completion of a physical therapy program approved by the American Physical Therapy Association, the Council on Medical Education and Hospitals of the American Medical Association, or the American Medical Association's Committee on Allied Health Education and Accreditation.
- 2-3 years of physical therapy experience preferred.
- Current CPR certification.
- Familiarity with computer systems, including internet, email, and electronic health record systems (Homecare Homebase).
- Valid driver's license and reliable transportation, with agency-required liability insurance.
- Additional Preferred Qualifications: Experience in home health settings, knowledge of relevant state regulations, and ability to manage online HR and training programs efficiently.
- Travel: Approximately 300 miles weekly, traveling 30-40 miles daily.
