Job Summary:
Outpatient Physical Therapist in South Bend, Indiana. Job Details:

• Location: South Bend, Indiana
• Schedule: Days, Shift Hours: 8 hours per day, Minimum Hours Per Week: 40 hours
• Required Qualifications: Degree in Physical Therapy from an Accredited School; 1+ Year of Outpatient Clinical Experience; CPR Certification; Active/applicable license for the State of Indiana
• Preferred Qualifications: Successful previous assignment with the agency or organization is highly desired; prior experience with Select Medical is nice to have
• Additional Information: Completes comprehensive patient evaluations, develops individualized treatment plans aligned with physician orders, maintains accurate documentation, participates in student training, and pursues ongoing professional development. Follows all regulatory and organizational standards to ensure quality and safety.
