Job Summary:
The Physical Therapist (Home Health) position is located in Garden City, New York. This role involves providing skilled physical therapy services to patients within a home health setting in accordance with physicians plans of care. Candidates are required to bring their license on the first day of employment. The position supports a Monday through Friday schedule, from 8:00 AM to 5:00 PM, with a standard assignment duration of 13 weeks.

Job Details:

• Location: Garden City, NY
• Schedule: Monday-Friday, 8:00 AM - 5:00 PM; 40 hours per week
• Float: Float rotation may be required.
• Required Qualifications: Successful completion of a physical therapy program approved by the American Physical Therapy Association, current unencumbered state license to practice physical therapy, 2-3 years of physical therapy experience preferred, current CPR certification, familiarity with computer functions such as internet navigation, email, and online HR services.

• Additional Information: This role requires providing physical therapy services directly within a home health setting, including assessing patient conditions, developing treatment plans, and collaborating with other healthcare personnel. The candidate must have reliable transportation and valid drivers license, and be comfortable using the PointCare electronic medical record system. The assignment begins on 06/22/2026, with weekly expectations of 30 units of productivity and travel of approximately 250-300 miles weekly. On weekends, there is a rotation every four weeks (Saturday only). No on-call duties are required.
