Job Summary:
Physical Therapist Home Health in Garden City, New York. This assignment involves providing skilled physical therapy services within a home health setting, aligned with physicians' orders and state-specific licensing requirements. The role emphasizes patient evaluation, treatment planning, and collaboration with other healthcare professionals to support patient recovery and independence.

Job Details:

• Schedule: Monday through Friday, 8:00 am to 4:00 pm, with a minimum of 40 hours per week
• Required Qualifications: Valid physical therapy license specific to the state of New York, completion of a physical therapy program approved by the American Physical Therapy Association or equivalent, 2-3 years of physical therapy experience preferred, current CPR certification, and proficiency with computer systems including internet, email, and navigation of software programs.
• Preferred Qualifications: Experience in home health care and familiarity with PointCare EMR system.
• Float: Float rotation may be required.
• Additional Information: The assignment covers the southwestern Nassau territory, requiring travel of approximately 250-300 miles per week. The contractor will operate under the supervision of a physician licensed in this state, with responsibilities including patient evaluations, developing treatment plans, and on-site assessments at least once a month. Reliably transportation, a current drivers license, and agency-required liability insurance are necessary.
